Once the information is received and processed, the Department will mail you:

  • Replacement serial plate
  • Partially completed Form T-22 Certificate of Inspection for assigned serial plate or T-88 for a rebuilt serial plate

After receiving the serial plate and Form T-22 or T-88, you need to:

  • Attach the replacement serial plate to your vehicle with rivets
  • Have a Georgia law enforcement officer complete and sign the T-22 or T-88 certifying that the replacement serial plate has been properly affixed to your vehicle
  • Mail the completed T-22 or T-88 to the MVD at the address below.  The MVD will issue a new title.
